Attached is a form that I used midway through my Positive Psychology course, and one that I used at the end. The forms gave me both quantitative and qualitative data. I adapted portions of these forms from some that exist on the TBL Collaborative website. Hope this is helpful.

Subject: TBL: Evaluations

I'm using TBL in a Geography undergraduate class, for the first time. While I'm enjoying the experience, I'd like to get useful student feedback on the process. My university does not have a standardised class evaluation form, so I'm curious about how people have gathered feedback on student experiences of TBL, and what kinds of questions you've found most useful.
