Hello, somewhere I saw a copy of a written appeals form but cannot find it now on my e-mail or in the Michaelsen, Knight, Fink (2004) book or on the website.  Can someone direct me to the right place or send me a copy?  Thanks - trying TBL for the first time this semester!
