I've created my own form, which requires teams to write their answer (see attached).  A team member is required to bring the sheet to me for grading after each decision making round. I mark it with a felt tip pen, which makes it very easy for me and the team to identify when they've made the wrong choice. They then go back to the group and we repeat the process until they've correctly answered each question. The process helps to add a little drama, as the rest of the group watches to see if I mark their paper.
