Marite,
I have attached a syllabus from a statistics course that shows an example of what an appeal should look like.
Not quite the same thing, but maybe it'll help.

Subject: Written Appeals Form?
Hello, somewhere I saw a copy of a written appeals form but cannot find it now on my e-mail or in the Michaelsen, Knight, Fink (2004) book or on the website. Can someone direct me to the right place or send me a copy? Thanks - trying TBL for the first time this semester!
